Application form contains basic information on:
1. In which session you wish to participate.
2. Name of the group (soloist).
3. Group leader.
4. Number of group members, among them: leader /s, girls up to 16 years old, boys up to 16 years old, women, men.
5. Organisation (art center, club) - name of the organisation, address, phone numbers etc.
6. Type of the art of the group (the soloist).
7. Information of production – authour, duration, name of the production.
8. Technical requirements information.
9. Lighting:

11. Art biography of the team (soloist)
12. Number of accompanying persons.

15. Excursions and Entertainment
16. Additional requisites.
17. Details of the person with whom we need to conclude a contract for accommodation, meals and transportation

Application form should contain complete and accurate information. Avoid abbreviations. Application certifies that the applicant accepts the terms of the International Youth Festival of Arts "Muses", which can be read from site http://www.muzite.org/.
The person who completes the form, agrees that personal data are processed and used by the organizers of the festival in connection with the contract.
